New Patients
What to bring/expect on the first visit:
- Plan on 60 to 90 min for the initial evaluation
- Insurance card
- Photo ID
- Therapy prescription from your physician
- Loose-fitting clothing for evaluation and exercises
- Please print and return the new patient form
